-1.1. Cultural Activities May Influence the Way We Think. A new learning model may explain how culture helped shape human cognition and memory. Cultural activities, such as the use of language, influence our learning processes, affecting our ability to collect different kinds of data, make connections between them, and infer a desirable mode of behavior from them. Essentially this is about learning to filter information to discard what is not considered necessary to perceive, how what is perceived is interpreted, and what range of acts are considered acceptable (PIA Glossary ʛ). This is a heuristic used in parademic, but one that has not been extensively covered in weekly notes for some time, and when it is referred to usually in the context of the language/narrative heuristics of SLA, 170716-.C.1, and linguistic relativity, 1.3↓. -1.2. Too Much Information Can Be a Good Thing. Psychologists examine how to package health recommendations. When does a person receive too much health information. What's the best way for health providers to convey information without consumers skipping over or forgetting key information. The answer lies in the goal of a specific health objective. One school of thought among health care providers is to give health information in small doses of two or three recommendations at once. Others argue that it's best to give patients the entirety of their options, so as to not skip out on something that may prove useful. The assertion is which approach taken depends on the nature of the recommendations. Presenting a large amount of information would be appropriate if the goal would be for people to remember a large amount of potentially interchangeable behaviors – research, analysis, long term strategy and development – but if the goal is for people to remember a complete set of important recommendations – decision makers who don’t have a deep understanding of the topic, or writers (plans, media, policy who need to produce simple, clear, concise material quickly and get the scoop credit. Both hopefully have good support from the lower status research, analysts and planners; and are willing to accept the support – then the best strategy should be to present relatively few recommendations. 1.3. Coming Face to Face with Disability Could End Supernatural Myth Making in Africa. Many people in rural African communities still believe that disability is caused by supernatural forces, curses and as ‘punishment’ for wrongdoings. Information on the medical causes of childhood disability are not widely available across communities in low income countries and understanding is generally poor. Albinism is explained by the mother having sex with a white man or a ghost. Epilepsy is widely thought of as being caused by evil spirits, or sometimes as a punishment for wrongdoing. Disability is often explained by things like extra marital affairs invoking a curse, witchcraft, supernatural forces such as demons or ghosts affecting the child, and the will of God. Curvature of the spine or limbs represented the effects of a curse, saliva production (a common problem with handicaps involving trouble swallowing and controlling muscles) was linked to demons and ill gotten financial gain. The different explanations represent a real mixture of traditional, religious and biomedical beliefs; while biomedical factors such as inherited conditions or antenatal care were increasingly talked about, these explanations did not negate other culturally based accounts, 1, 1.1↑. The underpinning all of these explanations is a desire to make sense of disability and, particularly for carers, to improve the given situation. When an explanation of wrongdoing or the presence of an evil force might result in a visit to a local Witch Doctor, a medical attribution might be followed with a visit to a medical centre. ¶ What tends to happen is that these types of cultural beliefs affect how individuals with disabilities view themselves and how other people see them. Attributing the child's condition to some form of malevolent preternatural force by reference to demons, evil spirits and witchcraft contributes to the view of disability as both undesirable and unacceptable (the stigma observed during and after Ebola, and not limited to Africa). People with disabilities in Africa have poor access to health provision, low school attendance, limited employment rates and low wages. More extreme consequences include neglect and abuse – with sexual abuse reported to occur at some time in the lives of 90 per cent of the population with learning difficulties. People with communication difficulties are at a high level of risk because they are less able to report abuse. We found that children with disabilities are often kept apart from the local community in restricted environments – contributing to the social distance between them. Although in many cases this is to protect them from abuse and keep them safe. The burden of caring for family members with disabilities also led to discrimination by association. And the stigma associated with people with disabilities is so great that it also extends to anyone trying to help, 1Hickcox↑. The implication being that anyone offering help would also give birth to a disabled child. But we found that the more people come into contact with disability, the better their understanding. Real life encounters with people who have disabilities can be a really positive step, so raising disability awareness in small communities can really help. Why Are Doctors under Using a Drug to Treat Opioid Addiction Two opioid replacement medications are currently approved for opioid use disorder: methadone, which under federal law must be dispensed from authorized clinics, and buprenorphine, which can be used to treat opioid addiction in the privacy of a physician's office, so long as the physician has the proper waivers. It was widely believed that allowing physicians to prescribe this drug in a primary care setting would increase the number of patients receiving treatment, but the number of physicians adopting this therapy has not kept pace with the magnitude of the opioid epidemic. Of 558 physicians in the United States only 74 indicated they did not have waivers to prescribe the drug, with about 25 saying they will never. The reasons for not obtaining a waiver included not wanting to be inundated with patient requests for buprenorphine and concerns about patients reselling their medication. More than half of the 484 respondents with waivers were not prescribing to capacity saying they have no time for more patients and insufficient reimbursement. Overall respondents indicated that the resources most likely to increase their willingness either to obtain waivers or prescribe to capacity were receiving information about local counseling resources, being paired with an experienced provider and receiving more continuing medical education courses on opioid use disorder. Government data published earlier this year estimated that 1.27 million people were hospitalized or sought help at an emergency room for opioid related issues in 2014, a 64 percent increase for in patient care and a 99 percent increase in emergency room visits compared with 2005. No idea if this is similar to physician reluctance to treat other “epidemics” that are likewise high risk (of failure) with low reward.
