-1. In a situation more serious than CV – that is one with rapid population decline, loss a labor force and expertise, widespread shortages due to failure of distribution and manufacture, fast economic decline, and likely political disruption – There needs to be consideration for shutdown. This is not the temporary shutdown seen during CV – though frankly it would have been more immediate, widespread, longer lasting and not as ad hoc – but a shutdown of physical and social systems.

            -A. There are five triage considerations of system shutdown: 1) Immediate and/or temporary of systems that are likely to be contributing to spread, virulence, and are crucial for continuity in the aftermath the biocalamity, or if maintenance of the system will divert from crucial systems needed for sustainment and recovery. 2) Some systems can be easily shut down, and will require either minimum maintenance for future restart, or can endure delayed maintenance for a long duration. 3) Other systems will require a more permanent shut down (mothballed), but done in a way that it will be possible to restart, to include related knowledge. These are not crucial for survival of society, but have alternatives, at the same time provisions need to be made for individuals who are dependent on the system as either employment or for their service needs. 4) In some cases there can be a determination that the system itself is no longer needed and will not be needed in the future. Is some cases these are institutionalized habits that are maintained even though no longer serving any purpose, and likely the most resistant to being shutdown. These can be left to deteriorate, though care needs be taken that as the system erodes it does not create health and environment hazards. 5) Last there are systems that may need to be destroyed as they are in themselves dangerous and cannot be secured without risk of serious harm or being used belligerently. Nuclear reactors would be an example.

            -B. When considering what category a systems triages under there are five steps: 1) Identify what is critical for life support now, and how those will change over time as a biocalamity continues. 2) Identify what is threatened, not specifically the health or environmental agent, but what threatens a system by either the myphetic agent(s), or by the containment and contol measures. This can be loss of a supply source, power, personnel, storage space, routes, etc. 3) Verify what vulnerabilities there are to an exisiting threat, for example a building and access to it may not be vulnerable to a flood, but drinking water to the building can be contaminated. 4) Do a risk assessment, the probability of an event happening, and the level of damage that will happen, analyze the risk. 5) Develop and apply mitigation and countermeasures. 6) Recognize that there are some things that are beyond a single company, or nation to solve. This is when collaboration is needed to mitigate, produce and implement solutions, and sharing resources that may not be useful to one group, but critical to others.

            -C. Like all planning, the is best done before need, implemented before harm, and then revised during the bioevent to assure that planning done without the feedback of an actual event still apply. Avoid using step by step or phase plans for biocalamity. That type of thinking tends to lead people to believe that one task is over and its time to move to the next. Biocalamities are living systems, or thrive on living systems, and tend to ignore human plans of what should happen.

CV06. Linguistics: Covid-19 Is Not a Pandemic. We must confront the fact that we are taking a far too narrow approach to managing this outbreak of a new coronavirus. We have viewed the cause of this crisis as an infectious disease. All of our interventions have focused on cutting lines of viral transmission, thereby controlling the spread of the pathogen. The “science” that has guided governments has been driven mostly by epidemic modellers and infectious disease specialists, who understandably frame the present health emergency in centuries old terms of plague. But what we have learned so far tells us that the story of COVID-19 is not so simple. Two categories of disease are interacting within specific populations—infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and an array of non-communicable diseases (NCDs). These conditions are clustering within social groups according to patterns of inequality deeply embedded in our societies. The aggregation of these diseases on a background of social and economic disparity exacerbates the adverse effects of each separate disease. COVID-19 is not a pandemic. It is a syndemic ^CV21. The syndemic nature of the threat we face means that a more nuanced approach is needed if we are to protect the health of our communities.
